What's known on the subject? and What does the study add? Penile lengthening methods remain a controversial issue. Surgical procedures of "lengthening phalloplasty" are characterized by poorly defined indications and an unacceptably high rate of complications, as recently outlined by a literature review, while non-surgical techniques are largely popularized by the media but often lack scientific evidence. In the literature we found only ten articles/abstracts of studies pertaining to the topic of our review. With our review, we aimed to explore whether non-surgical methods of penile lengthening may have some scientific background. We focused specifically on penile extenders, which among conservative methods are those whose efficacy is supported by some scientific evidence. It seems that penile traction devices should be proposed as the first-line treatment option for patients seeking a penile lengthening procedure. Penile size is a matter of great interest among men who are affected by 'short penis syndrome' or just believe themselves to have a small penis, even though the dimensions of the organ fall within the normal range. Surgical procedures of 'lengthening phalloplasty' lack standardized indications and carry a high risk of complications. Several non-invasive methods of penile lengthening have been described, such as vacuum devices, penile traction devices and penoscrotal rings; even 'physical exercises' have been popularized through the media. Most of these techniques, however, are not supported by any scientific evidence. We briefly analyse the efficacy and scientific background of such non-surgical methods of penile lengthening. It seems that penile extenders represent the only evidence-based technique of penile elongation. Results achieved do not seem to be inferior to surgery, making these traction devices an ideal first-line treatment option for patients seeking a penile lengthening procedure.

Introduction: Patients with buried penis, micropenis or functional penile dysmorphophobia seek for penile augmentation surgery. The commonly used methods for penile lengthening include inverse V-Y plasty of the dorsal skin, Z plasty of the ventral skin, suprapubic lipectomy and suspensory ligament release. These methods work on the penis from one side or another without calculation or prediction of the actual gained length. Patients and Methods: Between January 2000 and Mars 2007, a group of 53 patients complaining of short penis were managed by our new technique of penile lengthening. The new method is based on a geometric design that considers the penis as a cylinder required to be lengthened circumferentially. Our experience with penile deformations: incidence, operative techniques, and resultsBetween the initiation of the "Penile Deformity Clinic" in 1988 in our Department of Urology, and the end of 1992, 1,862 males aged 0 (less than 24 hours old) to 78 years old were examined. Five hundred of them were newborns, aged 0 to 5 days; they were examined to determine the incidence of congenital penile curvature. Two hundred and seventeen congenital curvatures were seen, and 102 of them were corrected surgically using four different corporoplasty techniques. From the 116 Peyronie's Disease patients, 37 were treated surgically, either by corporoplasty or prosthesis implantation. From the 29 iatrogenic penile deformities, all the result of previous hypospadias repairs or urethroplasties for stricture repairs, 18 needed surgical intervention, mostly for cosmetic reasons. Surgical outcome of congenital penile curvature treatment with corporal rotation technique2021 •
Congenital penile curvature (CPC) is a rare condition in men but has considerable impact on male sexual activity as well as their mental health. Many techniques had been proposed to manage this condition, however the corporal rotation technique had the advantage of preserving the penile length. Therefore, we conducted a study on 16 men with CPC who were treated with corporal rotation surgery to evaluate the outcome of this technique. The results showed that the mean degree of curvature was 51 ± 12.2 degree. The majority of patients had a moderate penile curvature (< 60 degree) and ventral curvature (75%). After the surgery, the residual curvature decreased significantly (from 51 ± 12.2 degree to 16.6 ± 6.34 degree with p < 0.001). We observed no remarkable changes in all penile dimensions including stretched penile length (13.6 ± 0.93cm before the surgery compared with 13.3 ± 0.89cm after the surgery with p = 0.01). An original mininvasive corporoplasty technique for penile curvature without circumcisionObjective: We describe an original minimally invasive penile plication technique with scrotal or infrapubic access, not requiring circumcision, for penile curvature of different severity and types. This technique can be used to correct both congenital and acquired curvatures, mono or bidirectional deformities. Corporeal plication for the treatment of congenital penile curvature2003 •
We evaluated the long-term outcome, effectiveness and patient satisfaction of corporeal plication for the correction of congenital penile curvature. A total of 25 patients with congenital penile curvature were included in the study. Mean age was 39 years (range 15 to 45). Patients with Peyronie's disease, and/or chordee associated with hypospadias or evidence of erectile dysfunction were excluded from analysis. All procedures were done on an outpatient basis using local anesthesia. The technique of corporeal plication consists of placing longitudinal plication sutures of 2-zero braided polyester on the opposing side of curvature until it is corrected during artificially induced erection.
